🧠 ĐẠI DỊCH SIÊU ÂM TUYẾN GIÁP: KHI HỆ THỐNG Y TẾ TẠO RA UNG THƯ:
I. MỞ ĐẦU: Một “đại dịch” đang âm thầm lan rộng
Bạn có biết rằng rất nhiều người khỏe mạnh sau khi đi khám sức khỏe định kỳ bỗng dưng trở thành “bệnh nhân ung thư”?
Và điều đó bắt đầu từ… một lần siêu âm tuyến giáp không cần thiết.
Trong vòng 10–15 năm qua, số ca ung thư tuyến giáp ở Việt Nam tăng nhanh một cách bất thường – chủ yếu là ung thư thể nhú kích thước nhỏ, phát hiện tình cờ qua siêu âm.
II. CHUYỆN GÌ ĐANG DIỄN RA?
1. Siêu âm tuyến giáp trở thành mặc định
- Trong các gói khám sức khỏe định kỳ ở cơ quan, công ty, xí nghiệp… siêu âm tuyến giáp thường được đưa vào mặc định, không dựa trên triệu chứng hay nguy cơ.
- Người dân không được tư vấn trước, không được hỏi ý kiến – chỉ đơn giản là “có trong gói khám”.
2. Người khỏe bỗng thành “bệnh nhân”
- Sau siêu âm, nhiều người được thông báo “có nhân tuyến giáp”, dù rất nhỏ (vài mm).
- Rồi bị chỉ định đi chọc FNA, làm sinh thiết, khám chuyên khoa.
- Cuối cùng, nhiều người bị mổ – mang nhãn “ung thư tuyến giáp thể nhú”.
**→ Điều này dẫn đến một loạt hành động y khoa không cần thiết: mổ, đốt nhân giáp, và theo dõi suốt đời mà không có lợi ích rõ ràng.
3. Nhưng tỷ lệ tử vong không hề giảm
Đây là nghịch lý lớn nhất:
- Số ca ung thư tuyến giáp tăng mạnh, nhưng tỷ lệ tử vong không hề giảm.
Điều đó cho thấy: phần lớn các ca này không cần phải được phát hiện – vì không ảnh hưởng đến sức khỏe và tuổi thọ.
III. ĐÂY CÓ PHẢI LÀ UNG THƯ THẬT SỰ?
Phần lớn các trường hợp là ung thư tuyến giáp thể nhú kích thước nhỏ (<10 mm) – còn gọi là *papillary thyroid microcarcinoma (PTMC)*.
Nghiên cứu tại Nhật, Hàn Quốc, Hoa Kỳ cho thấy:
- Rất nhiều u loại này không phát triển thêm sau nhiều năm.
- Không di căn, không gây triệu chứng.
- Người bệnh có thể sống cả đời mà không cần điều trị gì cả.
**→ Đây là loại ung thư có thể theo dõi chủ động (active surveillance), không cần mổ.
IV. TẠI SAO LẠI GỌI LÀ “ĐẠI DỊCH DO Y TẾ TẠO RA”?
1. Overdiagnosis – chẩn đoán quá mức
- Phát hiện ra một “bệnh” mà lẽ ra không cần phát hiện, vì không ảnh hưởng gì.
2. Overtreatment – điều trị quá mức
- Mổ cắt tuyến giáp toàn phần.
- Đốt nhân giáp bằng sóng cao tần.
- Uống iod phóng xạ.
- Phải dùng hormone suốt đời.
- Lo âu, chi phí, theo dõi kéo dài.
**→ Tất cả những điều trên xảy ra trong khi u giáp nhỏ đó có thể không bao giờ gây hại nếu để yên.
3. Lợi ích không thuộc về người bệnh
- Bệnh nhân chịu rủi ro và gánh nặng.
- Trong khi đó, hệ thống y tế hưởng lợi từ xét nghiệm – chẩn đoán – điều trị.
Đó là lý do vì sao các tổ chức y tế quốc tế cảnh báo:
📌 **“Không nên tầm soát tuyến giáp ở người không triệu chứng.”
(Ví dụ: USPSTF Hoa Kỳ, Hiệp hội Tuyến giáp Hoa Kỳ - ATA)
V. CHÚNG TA CẦN LÀM GÌ?
🔹 Với bác sĩ
- Không siêu âm tuyến giáp nếu không có triệu chứng hay yếu tố nguy cơ.

- Nếu phát hiện nhân giáp nhỏ, **đừng vội hoảng sợ**.
- Hỏi bác sĩ: có nguy cơ không? Có thể theo dõi thay vì mổ không?
- Tìm hiểu về “theo dõi chủ động” – một lựa chọn đã được chứng minh an toàn.
🔹 Với hệ thống y tế
- Rà soát lại các gói khám sức khỏe – không mặc định siêu âm tuyến giáp cho mọi người.
- Đào tạo lại bác sĩ siêu âm và bác sĩ nội khoa về chỉ định hợp lý, thậm chí cho carybác sĩ chuyên khoa ung thư.
- Ưu tiên sức khỏe người dân hơn là doanh thu.

That sounds amazing. As someone who’s been in private sector, now in SF public sector, this is something I wish Govt agencies could learn more about and begin to integrate. I could see, for example, the Capital Project & Direct Service public agencies benefit hugely from even basic baby steps in this area. Massive, disparate datasets but everyone seems to just be managing to stay on pace for the various mandated monthly, quarterly reporting, then overwhelmed by the annual or bi-annual budgeting process. It’s like painting the Golden Gate Bridge - Sisyphus, all over again.
