I don't want to be so dismissive that people who are finding uses for this tech won't pay attention to the important points about the limitations.
People *are* using this tech, some heavily, many probably in ways with pitfalls we won't see the worst results of until it's too late.
Saying "it's just fancy autocomplete" is basically true, but many people think "but autocomplete can't do --"
So, I really try to find ways to "get" this new tech.

Macquarie University's Huntsman Telescope (https://huntsman.space) has been knocked out by ants. Again.
The ants find their way into the electrical power distribution cabinet outside the telescope dome in large numbers, cause short circuits that trip the circuit breakers, and often get inside the breakers too, ruining them.
This happened in January 2018, December 2018, November 2019, December 2019 (took out several telescopes by tripping upstream breakers), February 2020, and now.
After the first few times this happened we looked into whether this was a common problem, and found that anecdotally some species of ants do seem to be attracted to electrical equipment (to the detriment of both the ants and the equipment) but there seemed to be no agreement as to why. 作为连接种植体和牙冠的部件，基台对种植牙后续使用性能起到重要作用，根据加工方式的不同，可分为成品基台和个性化基台。

2019年，高峰医疗获批了国内首款个性化基台，2022年又获批了国内首款个性化基台冠桥。在2021年之前，国内仅有3家企业生产的3款持证个性化基台，而到2024年，一年内新获批的个性化基台及个性化基台桥架就达69个（仅1个产品为进口），此类产品数量的暴涨，成为种植修复产品激增的最直接原因。

截至2024年底，已有96家企业的120多款个性化基台及个性化基台桥架产品获批，绝大多数为国产。

个性化基台产品的井喷，并非没有理由。
首先当然是患者需求使然。国内知名种植修复耗材企业高管鹿鸣（应受访者要求，为化名）谈到，当前中国老龄化形势严峻，由于几十年前大众牙健康意识不足，导致目前老龄人群口腔基础情况不理想，医生面临的临床困难会更多，对个性化种植的需求呈爆发式增长。
由于成品基台是标准化生产，同型号成品基台的形状、尺寸等完全相同，难以满足临床复杂的修复状态和患者的个性化需求。“一些患者骨条件不理想，若使用成品基台就需要大量植骨，若不植骨就可以考虑使用个性化基台。”鼎植口腔总裁黎强表示。
当前，数字化技术已被广泛应用于口腔种植领域，其中也不断加深在个性化基台设计制造中的应用。
个性化基台需根据患者的缺牙间隙、咬合关系及种植体植入的三维位置来设计加工。在设计制作个性化基台时，需利用口腔扫描系统扫描模型，获取种植体与邻牙、颌牙的空间关系，并利用CAD进行可视化设计，进而获得良好的外形、形态及牙龈轮廓等。数字技术在精确的扫描数据采集、高效的精密加工等环节都起到了重要作用。
从拿证企业来看，创英医疗、威高洁丽康、百齿泰、大清西格等种植体企业在丰富产品线的过程中，已拓展了个性化基台产品；同时，诸多义齿加工公司、综合类口腔耗材制造企业，也纷纷布局个性化基台产品。
在鹿鸣看来，近年来牙科技工所规模效益显现，使得个性化基台在加工制造环节具备了更充足的专业能力。“规模化的牙科技工所具有投入数字化生产设备的实力，很大程度上促进了个性化基台的发展。”
此外，个性化基台曾经历了一个无序混乱阶段，随着监管趋严，如今，持证、合规生产销售成为必然。
快速普及趋势下的关注点
“个性化基台已在临床上迅速普及。”这是黎强的直观感受。
目前，个性化基台也已在临床应用的多个环节显现出积极价值。例如，前牙缺失患者开展个性化修复基台和即刻种植术治疗，并联合护理干预措施，能够提升种植修复质量，获得满意的美学效果，改善口腔健康状况。
也有研究指出，个性化基台凭借其良好的穿龈形态及良好的固位效果，减少了食物嵌塞问题，提高了患者的治疗满意度。
不过，个性化基台充分发挥临床价值的前提，是精准的接口匹配、精细的个性化加工。
个性化基台的下部接口部分（即基台与种植体的接口部分），根据适配种植体的不同，分为多种接口类型，如三角形、内六角、十字锁合、八边形等。下部接口部分应与种植体的接口形态、大小、锥度相匹配。即使是相同的种植体品牌，往往也会有多种不同的种植体接口设计。
基台与种植体连接界面是将咬合力从基台传导至种植体的部位，二者配合紧密有利于均匀地分散咀嚼力，确保基台与种植体系统的长期稳定。由于需要与不同品牌、不同型号的种植体连接使用，个性化基台与种植体的配合性能尤为重要。若个性化基台与种植体接口不匹配， 则会导致如螺钉松动、断裂、配合处磨损、配合间隙过大等问题；一旦基台与种植体的间隙过大，还会引起细菌微渗漏，增加种植体周围炎和牙槽骨吸收的风险。
因此，个性化基台与各大品牌种植体接口的精确适配程度，一直是基台厂商重点关注内容，也是影响个性化基台品质的关键环节。
作为较早投入市场的产品，高峰医疗的个性化基台已经建立起强大的数据库，且加工精度高、匹配多种接口。
创英医疗的个性化基台兼容众多种植体系统，采用与种植体同等的生产工艺和质检标准，接口与种植体制造商原厂基台精度相当。
贝施美个性化基台适配超100种植体系统型号，可实现穿龈形态、穿龈高度、基台直径、修复高度、修复角度等参数个性化定制，CNC数控多轴设备精密加工，可以使个性化基台与原厂植体、替代体高度匹配。
个性化基台的上部为个性化加工部分，尽管可根据患者口腔情况定制，但每款产品可进行个性化加工都只能在一定参数内，包括切削角度、穿龈高度、最薄切削部位厚度等。
鹿鸣坦言，个性化基台的力学强度要求比成品基台更高，加工时如果只追求切削效率，而忽略了应有的角度和范围，可能导致力学强度不足，长期结果则是出现基台断裂的情况。“这是未来个性化基台需要加强品控的一个重要方面。”
此外，个性化基台也通过产品设计或工作流程优化，提升治疗效率。
睿植康以个性化基台及螺钉、可切削基台柱及螺丝为代表的修复系列产品，强调个性化设计，且相对传统基台简化了治疗流程，无需埋入牙科排龈线和定制印模帽。
柯润玺医疗的个性化基台在保证品质的同时，也可以适应不同病例，配合扫描杆、口扫仪器，提升口腔临床的操作效率，节省临床时间。
洋紫荆牙科则将个性化基台与种植冠并联同步制作，节省订购、物流及调磨时间。
个性化种植，不只是基台
种植修复各类产品的发展并不是独立的。
过去两年来，得益于种植牙集采政策的落地，国产种植体已经历了一轮快速飞跃。
鹿鸣认为，一方面，国产种植体正在经历从有到全的阶段。“10年前，各大厂家普遍都只有一款种植体系统，投入市场后持续验证产品的功效和品质。之后，各厂家不断补全自己的产品线，目前许多厂家从临床需求出发，已布局多款种植体系统，可针对不同适应症，满足半口种植、全口种植、骨条件不理想患者等的多样化需求。”
另一方面，流通渠道变革倒逼厂商的产品升级迭代。据了解，种植牙集采后，由于利润空间被压缩，部分经销商退出市场，原本由经销商为医疗机构提供的培训、学术交流减少。因此，厂商不断从医生使用便捷性出发，优化核心产品、配套手术工具等的设计，缩短医生学习新产品的时间，帮助医生提高手术结果的可预见性与成功率。
更重要的是，国产产品获得了患者认可。目前，种植体厂商越来越重视患者回访，尤其是产品上市时间在5-10年的企业，种植体植入后已达到一定年限。“根据我们的回访，患者普遍对产品质量认可，后续国产产品还需进一步加强在医生中的品牌影响力。”鹿鸣介绍。
作为同样受益于集采的产品，个性化基台是否也能延续上述路径？从获批节点来看，目前大多数合规新产品在实际临床中的应用时间较短，长期稳定性仍有待持续观察。
不过，可以肯定的是，针对患者的个体状况，种植牙的个性化趋势不只是个性化基台，而是成体系地创新、满足患者个性化需求。
黎强以种植体为例谈到，在做即刻种植、不植骨种植时都可能用到新型产品，比如穿翼板的种植体、穿颧骨种植体，但这类特殊种植体还存在空白，需要医疗机构、上游企业共同研发攻克，满足中老年少植骨或不植骨时的种植需求。
相信在种植耗材企业、加工企业、医疗机构、监管部门各个参与方共同努力下，未来大众不只是对种植牙与牙健康有更全面的认知，也不只是能获得更普惠产品，更能轻松获得最适合的个性化种植修复解决方案。
